• Title : A Case of Dual-Zone Technique Combined with a Static Guide for Immediate Implant Placement and Provisionalization in the Esthetic Zone

• Abstract :

  Purpose:

   This case report presents the management of an esthetic zone edentulous site using dual-zone bone grafting combined with digitally guided immediate implant placement and restoration. The objective is to summarize key clinical protocols for achieving predictable esthetic outcomes in such scenarios.

   Materials and Methods:

   A comprehensive preoperative evaluation was performed, including CBCT and intraoral scans, to create a virtual patient model. Implant positioning followed a prosthetically driven approach and the 3A2B principle. A static surgical guide and silicone template for provisional restoration were fabricated. At the first appointment, the implant was placed using the digital guide, with dual-zone grafting applied to reconstruct both bone and soft tissue compartments. An immediate provisional crown was delivered on the same day.

   Two months post-surgery, the gingival margin was slightly coronal to the contralateral tooth. Gradual soft tissue sculpting was performed by adjusting the provisional crown over three months before final restoration.

   Conclusion:

   While immediate implantation in the esthetic zone offers significant advantages, it cannot fully prevent ridge contour collapse or mucosal recession caused by post-extraction bundle bone resorption. To manage such risks, GBR is often combined with techniques like connective tissue grafting (CTG), socket shield technique (SST), or the dual-zone approach. This case achieved satisfactory esthetic results through meticulous planning, guided 3D implant placement, immediate provisionalization, and staged soft tissue conditioning. It provides a reference for managing similar clinical challenges.
