Title : A case of immediate implantation and immediate restoration with alveolar ridge augmentation via vestibular incision in the aesthetic zone
Abstract : This case involves a 39-year-old female patient whose two maxillary central incisors, previously restored, had developed discoloration. The procedure was performed using a fully digital guide for immediate implant placement and provisionalization.
Implant therapy in the esthetic zone carries high esthetic risks and requires a prosthetically-driven treatment plan, thorough preoperative preparation, and precise surgical execution. In this case, the patient presented with an intact alveolar ridge crest but perforation in the apical region. To avoid potential bone resorption at the crest that could result from extensive flap reflection, a vestibular incision was made, followed by bone grafting, preserving the intact gingival emergence profile. A digital surgical guide was used preoperatively, and a provisional crown was fabricated in advance to avoid a toothless period after surgery. The provisional restoration helped maintain the gingival margin contour, contributing to a favorable esthetic outcome.
