  • Abstract : Evolution of implant therapy in aesthetic areas now focusing on the challenges and advancements to achieve consistent and long-lasting results. A more refined approach was developed, prioritizing careful planning, soft tissue management, and the use of provisional restorations. The planning process began by defining the final soft tissue margin line and emphasize the importance of visualizing the bigger picture of aesthetic. The peri-implant aesthetic management was guided by three main principles: soft tissue margin management, provisional restorations, and periodontal-prosthetic fundamentals. Soft tissue margin management involves either preserving or re-establishing tissue, depending on the tissue type and clinical situation. The choice of provisional restoration (whether passive full-contour, active full-contour, or sub-contoured) depends on the desired tissue movement and the design of the emergence profile. Key anatomical areas such as the crestal, bounded, and aesthetic zones (EBC concept) must be shaped carefully to ensure tissue stability. Clinical case, 36 years old male patient want to replace his missing anterior teeth with dental implant supported restoration. For this case, margin reestablishment therapy was needed, along with active full-contour provisionalization to reposition the soft tissue margin. A gingivectomy was performed before guided implant placement, ensuring the correct fixture depth and labial space. Grafting materials were used, and immediate provisional restorations followed the EBC concept was inserted. Postoperative results showed gradual soft tissue maturation, with final zirconia restorations enhancing aesthetics and function. This case underscores the synergy of planning, biological principles, and prosthetic design in achieving long-term aesthetic success. Conclusion: Plan before you plant the implant by designing the ideal final margin line in advance, build the ideal emergence profile by following the EBC concept of provisionalization, and apply perio-prosthodontic fundamentals to shape outcomes that harmonize in aesthetics, function, and biology.
