Title : Beyond Bone : 3D Soft Tissue Augmentation for Esthetic Implant Success
Abstract : The esthetic zone presents one of the greatest challenges in implant dentistry, particularly when both vertical and horizontal tissue deficiencies compromise the final outcome. While Guided Bone Regeneration (GBR) has traditionally been the standard of care, recent evidence highlights the pivotal role of soft tissue in achieving long-term stability and esthetics. This case report describes the management of a maxillary lateral incisor with severe tissue deficiency using a soft tissuedriven approach. A 3D augmentation was achieved through the Double Connective Tissue Graft Platform Technique, as described by Giovanni Zucchelli, allowing for simultaneous vertical and horizontal gain without bone grafting. The surgical protocol, healing sequence, and final esthetic integration are presented, demonstrating how soft tissue manipulation can replace or complement GBR in selected cases. This clinical experience underscores the concept of Soft Tissue First as a predictable strategy for peri-implant stability and natural esthetics, aligning with the current paradigm shift in implant dentistry.
