• Abstract : Digital technology is reshaping the way we approach full-arch implant rehabilitation. This presentation explores the All-on-X revolution, where precision and efficiency are achieved through the integration of fully digital workflows. By combining CBCT imaging, intraoral scanning, and R2GATE planning software, clinicians can design restorations from a prosthetically driven perspective, optimizing implant placement and reducing overall chair time by nearly half. The lecture focuses on four major clinical challenges in complex cases: prosthetic design, three-dimensional implant positioning through guided surgery, bone regeneration, and soft tissue management. Each stage is addressed within a systematic protocol that emphasizes accuracy, predictability, and long-term biological stability. A high-risk caseadvanced periodontitis in a heavy smokeris presented to illustrate how preventive assessment, microbiological and blood testing, and peri-implant risk management can be successfully incorporated into treatment planning. The workflow demonstrates how immediate loading under sedation, combined with CAD/CAM manufacturing of monolithic or hybrid materials, allows patients to achieve both function and esthetics within 24 hours. Beyond surgical precision, this approach highlights the importance of personalized prevention, patient education, and careful restorative design to minimize biological complications such as peri-implantitis.
