  • Abstract : The preservation of esthetics and function in the anterior maxilla remains a clinical challenge, particularly when tooth extraction is immediately followed by implant placement. This case presentation describes a multidisciplinary approach combining several advanced techniques to achieve predictable outcomes in a compromised site. An immediate implant was placed in the premolar region using the socket shield technique to preserve buccal bone integrity and soft tissue contours while the SPOT (Selective Preservation of Tooth) protocol was applied for apex removal and easy shield preparation and to enhance biological healing and osseointegration. Septum expansion and sinus lift with osseodensification techniques were performed to optimize bone volume and ensure implant stability in the compromised socket in maxilla. Guided osseodensification was utilized to enhance bone density and primary stability, while digital surgical guides allowed precise implant positioning. Following placement, the implant was immediately restored with a provisional crown to maintain soft tissue architecture and patient function and aesthetics. This case highlights the synergy of contemporary surgical and prosthetic protocols in managing challenging extraction and implant scenarios. By integrating socket shield, SPOT, sinus management, and guided techniques, clinicians can minimize invasiveness while maximizing esthetic and functional outcomes. Immediate loading further enhanced patient satisfaction by reducing treatment time and improving quality of life. The presented case underscores the importance of digital planning and biologically driven surgical protocols in modern implant dentistry.
