Title : Immediate implant placement after removing an impacted canine
Abstract : A 28-year-old male patient presented our office with an impacted upper right canine. There was a follicular cyst around the tooth and the upper premolars were also failing. The first premolar had a root resorption and the second had a penetrant caries. After careful removal of the affected teeth, an immediate implant placement was performed at the second premolar with simultaneous socket management of the premolars. Since the available bone was very limited, the insertion torque was not high enough to continue with immediate provisionalization. However the patient had to wear a removable provisional (Essix retainer) only for 3 months. During the second phase, additional augmentation was also performed. After 6 months, the second implant at the position of the canine (originally first premolar) was placed, and a small connective tissue graft was also harvested from the palatal flap for additional soft tissue augmentation. 3 months later, the provisional was transformed into two separate crowns to shape the soft tissue. Multilayered zirconia crowns were fabricated after digital impression. The patient is still very satisfied with the overall esthetic outcome of the treatment and that he had to use a removable provisional for only a few months.
