  • Title : Peri-implant soft tissue optimization - Using apically positioned flap with periosteal fenestration

  • Abstract : A key factor in maintaining peri-implant health is the creation of an environment that allows for effective oral hygiene control. Shallow vestibular depth can make it difficult for a toothbrush to reach the junction between the implant prosthesis and the gingiva. This condition is also strongly associated with a lack of keratinized mucosa (KM), which contributes to a soft tissue environment that is less resistant to inflammation.

     Free gingival graft (FGG) is a reliable method for KM augmentation, but it carries critical risks of complications. On the other hand, an apically positioned flap (APF) is a simpler alternative but is often accompanied by significant relapse. To overcome this limitation, we incorporated a periosteal fenestration into the APF procedure. According to a recent study published in the Journal of Clinical Periodontology, the area where fenestration is performed heals with dense, scar-like connective tissue, creating a barrier that prevents the upward migration of the underlying movable mucosa.

      In this presentation, clinical applications of APF with periosteal fenestration in different time point of the implant treatment phase are introduced. Peri-implant soft tissue optimization by the means of APF with periosteal fenestration can be applied in convenient and minimal-invasive manner during implant surgery and, even after prosthesis delivery.
