  • Title : Management Strategies for Defect of the Maxillary Sinus Floor

  • Abstract : In the posterior maxilla, clinical situations such as severe periapical or periodontal lesions combined with maxillary sinus pneumatization, or retreatment of implants previously placed through the sinus floor, may lead to the destruction of the sinus floor bone following tooth extraction. Although this phenomenon is not uncommon in daily practice, clear clinical guidelines for its management remain insufficient. Such defects present significant challenges in both implant placement and bone augmentation procedures, often complicating surgical planning and long-term stability. The present presentation aims to emphasize the importance of early recognition of sinus floor defects at the extraction stage and the necessity of infection control, particularly in cases with odontogenic sinus involvement. A decision-making framework is proposed according to the size, location, and condition of the defect. Treatment options include prosthetic approaches to avoid implant placement by using pontics or modified implant-supported bridges, direct sealing of the defect when an implant can be placed in an ideal position, and staged approaches involving guided bone regeneration with particulate grafts or reconstruction with block bone grafts when the defect is extensive or poorly positioned. In addition, attention is given to the biological limitations associated with poor healing of soft tissues and the risk of epithelial ingrowth or mucosal adhesion at the defect site. Finally, recent literature is reviewed to provide evidence regarding long-term prognosis, risk factors, and surgical considerations, thereby offering a structured rationale for clinical decision-making in cases of sinus floor bone destruction.
